1. Two or more convictions for speeding >55 mph within 12 months of each other
2. At least one reckless driving conviction AND at least one conviction for speeding > 55 mph within 12 months of each other
3. At least one aggressive driving conviction AND at least one conviction for speeding > 55 mph within 12 months of each other
4. One conviction of speeding >80 mph in a <70 mph zone
5. One conviction of speeding over 80 mph in a less than 70 mph zone
